For every client including models, I always complete a record card. Not only is this part of my insurance but it also ensures that I can ask any questions, make notes and ask the models/clients to sign that the information is correct before every treatment/shoot. With regards to release forms I have two different ones. I have a photo release for clients and for models I have a model release form, as seen below. These documents basically gives me a copyright on the photos, allowing me to use the photos taken of them with out having to pay any further money. I can then use them on my website, advertising, blog etc without having to worry about the model becoming famous and suddenly wanting her photos back. My model release form is below.
I got the original info for a model release form from The Royal Photographic Society who exist to promote photography and image-making and to support photographers in realising their potential, irrespective of their level of knowledge, equipment or skills. It has an international membership and anyone who has taken a photograph can get their advice.
I downloaded my original Model Release Form from this page and then spoke to a couple of photographers who helped me tweak it to suit my needs.
I will NOT be uploading the completed forms to my blog site for Data Protection reasons however I will be using both of these forms for every shoot.
